Tournament Registration Form
Only ONE person from EACH Doubles team needs to fill out the form to
REGISTER for the tournament!
PLEASE READ before you complete STEP 2!
Players who have already registered/paid for the DOUBLES LEAGUE prior to the tournament skip to Step 3. Payment for the tournament is included in your Doubles League Registration/Payment.
Players who HAVE NOT registered previously for the Doubles League must make payment and complete STEP 2 for the tournament.

Waiver & Release Form
GCTA requires that EVERY person MUST fill out a "Waiver & Release" form. Once you have filled out the form it is good for any and all GCTA events for the 2022 season. If you have NOT filled one out, click on form, print/fill out, and bring to the tournament.
